Anxiety Level:

Please rate your level of anxiety, with 0 being the best and 10 being the worst.

Depression Level:

Please rate your level of depression, with 0 being the best and 10 being the worst.

Sleeplessness:

Please rate your level of sleeplessness, with 0 being the best and 10 being the worst.
